Throughout the year, students or parents are asked, at times, to fill out various records, permissions, or application forms. In addition to the hard copies sent home, we are compiling a list here for your convenience. Click on the links below to download PDF copies of these forms.
AUP/Media Permission Form: The first part of this 2-part form contains the "Acceptable Use Policy" for using the Internet at Greenwood Schools. Students understand by signing this form, that they are to use the Internet for educational purposes at school, and the parents' signature is required for their students to be able to use our Internet. Internet research is required as part of the Indiana Standards, so a completed AUP on file is a must. The second part of this form is the media permissions form. By signing this, parents are agreeing that their student's picture and/or name may be included in Newsletters, our website, bulletin boards, local newspapers, or other local media. (Note: Page 1 is to be filled out, and page 2 is a duplicate for your records)
Download a Free / Reduced lunch application. Print out and complete this form in order to apply for free or reduced lunches as well as textbook assistance. After you have completely filled out this form, please return it to Mr. Sutton at the middle school. (523 S. Madison Ave. Greenwood, IN 46142 889-4047 ext. 313) Parents or guardians with children who are members of a currently certified Food Stamp or Temporary Assistance for Needy Families (TANF) household may sumbit an Application for Meals with a valid case number, in lieu of reporting income. Food Stamp and TANF case numbers have 10 digits that start with a 1 or 0. Hoosier Works card numbers can not be used to qualify families for free or reduced meals.
Bully Free Parent Form: Bullying has become a concern in many communities in the United States. Whether its at the school, in the neighborhoods, or over the Internet, it is counterproductive to all of our citizens. In this form, you as a parent, can formally say that you support an environment that is as safe as possible, and will communicate with the school about bullying.
